*A separate application must be filled out for each child in the family who wishes to attend.

    Enrollment is confirmed only with written verification from the Director.

    We try to accommodate all students as the schedule and program capacity allows. Five day applicants will receive priority. Schedule variations will be accommodated as space allows.

    *Documentation of physical examination and immunization in accordance with school health requirements and lead poisoning screening in accordance with public health requirements will be kept in your child’s school file.

    Please submit the application form to Shaloh House. The deposit is $500 for all classes upon acceptance. The fee is non refundable and non transferable. A portion of the fee ($300) will be applied to your tuition.
